Comprehensive 24-hour service:  Community Lifeline provides a comprehensive 24-hour care service in the client’s own home, tailored to their specific needs.  By utilising the latest technology systems to monitor smoke, intruders, inactivity, or the use of a pendent/pull cord alarm, just to name a few, we can offer a response service to a call at any time, day or night.

Dementia Care Service:  We specialise and provide support for Dementia clients so that they can remain in their own home of familiar surrounds.  We support the partners and relatives so that they can understand and support their loved ones and enjoy life to the fullest.

Working with other professionals:  Our professional carers work closely with social services, district nurses, GPs, Occupational therapist and other Multi Agency professionals, as well as the client’s family and friends.  We can therefore provide a flexible, integrated service that supports the client in their own home.

Assistance with daily routine:  We provide a flexible service, through Person Centred Careplan, that gives you choice and control. Some of the examples of service we provide are

  • Assisting with bathing, showering, continence care and dressing.
  • Assist with medication from reminder, monitoring, administering and collecting of prescriptions.
  • Helping to plan a well balanced nutritional diet, preparing meals and feeding.
  • Domestic duties from cleaning the house, laundry, ironing, changing bedlinen and many more.
  • Assist with shopping, collecting pensions, daily light walking and exercise.
  • Companionship and activities.

 

 


Out of hours Service and Rapid Response:
 You can contact us 24 hours a day if there are any emergencies or change in plans, such as family taking you out for the day. We can provide ‘out of hours’ support to clients and family carers in circumstance such as when the family carer is taken to hospital.  If required, our out of hours service can contact a family member, friend or neighbour on your behalf if required.

Hospital to Home and Reablement:  We can provide time limited service, from two to six weeks, when clients return home from hospital after illness, surgery, accident, stroke or long stay in the hospital. The service provides, through Person Centred Careplan, help to improve mobility and daily living tasks allowing them to regain their skills, so that they are able to remain at home with minimum or any support or assistance.

Additional services:  The client may have a need of a variety of services daily/weekly or one off, such as being accompanied on day trips, visiting friends and relatives or travelling abroad. Community Lifeline services can provide the appropriate level of service for all requirements.
